Prepare at your current registrar
The four things that most often make a transfer fail.

Unlock the domain
Turn off "domain lock" in your current registrar's panel.

Get the EPP code
Also called the auth code, usually sent to the owner's email.

Domain older than 60 days
Newly registered or recently transferred domains cannot be transferred again yet.

Owner email is active
The current registrar may ask for confirmation by email.
Track it until it is done
You can follow the transfer status in the client area. While it is still in progress, the transfer can be cancelled.
